In a fast paced environment, the Case Manager will regularly work with clients coming from mental health, substance abuse, homelessness and recently incarcerated. As a Case Manager, you will help them get back on their feet and live more independent lives with the goal of helping them find long term housing.

Intake process:

  • Completes an initial needs assessment and acts upon critical needs appropriately and immediately

  • Orients clients to the program, requirements and consequences

  • Assures that basic nutrition and hygiene needs are being met

Case plan:

  • Develops a comprehensive client-driven case plan with both short-term and long-term goals identified

  • Completes a timeline and measures for each goal

  • Monitors progress towards goals in regularly scheduled weekly sessions

  • Evaluates and adjusts case plans as needed and provides written warnings with consequences if satisfactory progress is not being met

  • Empowers clients to become involved in their own planning and goal setting

  • Refers clients to appropriate resources to assist with meeting goals

  • Assures that specific HUD program goals are consistently being met and provides extensive case work to foster transition from homelessness to permanent housing

  • Designs, coordinates and implements Life Skills, Tenancy 101 and other training program aimed at learning and practicing life skills and decision-making

  • Establishes and coordinates day and evening programs with staff for clients

  • Treats all clients, visitors and employees with caring, kindness, respect and dignity

  • Adheres to policies, procedures, and code of conduct

  • Maintains strict confidentiality of all information.

  • Adheres to the policies in the use of computer technology and all telecommunication devices

PREFERRED QUALIFICATIONS

  • 1 year experience as a Case Manager

  • Experience working with the substance abuse, mental health with dual diagnosis population

  • The ability to use a variety of de-escalation tactics to calm down clients  Effective written and verbal communication.
